Put her in your shoes by asking her if she just doesn't have any respect for you or if there is something that she has going on with someone else that won't allow her to get away and spend time with you like she used to. I agree with the meeting at Applebees at 7 idea. And leave it at that. (This is all assuming that she shows up). If she doesn't show, call her and leave her a message thanking her for making it clear to you how much respect she has for you and for the wonderful dinner you had alone. At that point, I would say to her, "When can you meet me?" Let her make the plans but make sure that she gives you a day and time. I feel like if you put her on the spot, then she'll either agree to meet you or say that she has plans.
